The Greater Humboldt Park Community of Wellness

The Community of Wellness is a coalition of greater Humboldt Park hospitals, clinics, non-profit social service organizations, the Park district, YMCA and others, who come together to ensure the community has quality health services that are available to everyone.

The Greater Humboldt Park Community of Wellness defines health based on the World Health Organization Declaration of Alma-Ata, International Conference on Primary Health Care, 1978:
 
“Health, which is a state of complete physical, mental and social wellbeing, and not merely the absence of disease or infirmity, is a fundamental human right and that the attainment of the highest possible level of health is a most important world-wide social goal whose realization requires the action of many other social and economic sectors in addition to the health sector.”
Our MISSION is to facilitate a comprehensive vision of wellness by working collaboratively with a broad array of people and organizations to leverage their complementary strengths and capabilities to solve community health problems.
 
Our VISION is to create a Community of Wellness in Greater Humboldt Park, where health is an aspect of all community development. Residents will be informed about and have ready access to culturally sensitive and affordable healthcare and services, and can take a pro-active role in their family’s health.
 
Our GOALS for achieving the vision are to:
  • Involve the community and people experiencing barriers to care in identifying and implementing strategies that ensure healthy citizens.
  • Serve as a conduit between community goals and needs and organizations working to fulfill those needs through a process of public awareness and education.
  • Identify and build on community assets to highlight the Community of Wellness and ensure culturally competent, efficient and effective responses to health needs.
  • Strive to make health care affordable in the community.
  • Provide a forum for the formulation, dissemination and advocacy of health policy from a concrete local perspective that can vigorously inform city, state and national efforts.
